Direct Support Professional (DSP)

Job Title: Direct Care Professional

Position Summary:

As a Direct Care Professional, you will play a pivotal role in supporting individuals within a Community Living Group Home setting. Your primary responsibilities will include scheduling and accompanying individuals to medical appointments, facilitating community development learning activities, and implementing activities of daily living to achieve the goals of the Community Living Group Home. Proficiency in Microsoft Office applications, particularly Excel, is essential for success in this role. The ideal candidate will be compassionate, reliable, and dedicated to enhancing the quality of life for individuals under your care.

Key Responsibilities:

1. Medical Appointment Coordination:

  • Schedule and coordinate medical appointments for individuals under your care.
  • Ensure all medical records and documents are up to date and available for appointments.
  • Accompany individuals to medical appointments and provide necessary support.

2. Community Development Learning Activities:

  • Plan and facilitate activities that promote social and community integration for individuals.
  • Organize and lead group outings, workshops, and educational events to enhance community participation.
  • Foster an inclusive and supportive environment that encourages social interaction and personal growth.

3. Activities of Daily Living (ADL) Implementation:

  • Assist individuals with ADLs, such as personal hygiene, grooming, meal preparation, and housekeeping.
  • Encourage independence while providing necessary support based on individual needs.
  • Create and maintain a safe and clean-living environment for residents.

4. Individual Goal Achievement:

  • Work with residents to establish and pursue their personal goals within the group home setting.
  • Keep detailed records of progress and setbacks, adjusting care plans as needed.
  • Collaborate with the care team to develop individualized care plans.

5. Prioritizing Safety, Health, and Well-being:

  • Always ensure the safety and security of individuals.
  • Promote a healthy and supportive living environment that contributes to the overall well-being of residents.
  • Be vigilant in identifying and addressing any potential safety concerns or health issues promptly.

6. Administrative Tasks:

  • Maintain accurate records, including medication logs and incident reports.
  • Keep track of inventory and supplies, ordering items as needed.
  • Document and report any significant changes in residents’ conditions.

7. Communication and Teamwork:

  • Communicate effectively with individuals, their families, and colleagues.
  • Collaborate with other team members, including nurses, therapists, and supervisors.
  • Attend team meetings to discuss and plan care strategies.
