Community Residential Services, provided through our Community Living – Group Home program, support individuals in developing living arrangements that reflect their personal needs, preferences, and goals. We help individuals build meaningful connections that allow them to become valued members of their community.
We serve individuals requiring varying levels of support—from occasional drop-in assistance to 24/7 nursing care. Services are delivered in a variety of settings, including homes owned or rented by the individual or family, as well as apartments, houses, or townhomes leased by the agency. The most common arrangement consists of two or three people living together with full-time staff support.
Individuals who require twenty-four-hour nursing care are supported in medically supervised homes staffed by registered nurses (RNs), licensed practical nurses (LPNs), certified nursing assistants (CNAs), and Direct Support Professionals (DSPs). Services include housing, transportation, individualized training, assistance with household management, support with medical and financial needs, personal care, recreational activities, and coordination with community resources.
